Resource navigation, case management & care coordination
are terms that are used interchangeably to describe activities that seek to
use community supports, resources & wrap-around services to:
improve the client's health and well-being,
enhance the client's ability to advocate for themselves,
reduce the duplication and fragmentation of services, and
optimize the use of resources that support stated goals.
At Elk, our Resource Navigators & Case Managers focus on helping our clients access and navigate the system, overcome barriers, and advocate for their rights and entitlements. Care coordination is focused on facilitating the integration and collaboration of services, ensuring the continuity and quality of care, and promoting the client's self-management and independence.

For Elk, resource navigation/case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet our client's and family’s comprehensive behavioral health needs - through communication and available resources to promote safety, quality of care, and cost effective outcomes.

Resource navigation helps us identify appropriate providers and facilities throughout the continuum of services, while ensuring that available resources are being used in a timely and relevant manner.
Care coordination is a systematic approach that Elk takes on to ensure that the client's needs and preferences are being met across the continuum of care, and that the information sharing amongst the client, family, and care providers are meaningful and timely.
Our care coordinators work with the client and the providers to develop and implement a care plan that aligns with the client's goals and values.
