1. PURPOSE

​

At Elk Meadow Enduring Connections, Inc. (Elk), your feedback matters. If you're dissatisfied with our services, you have the right to file a grievance. This policy defines how to submit, process, and resolve grievances in compliance with standards set by the Colorado Behavioral Health Administration (BHA). Per Colorado Revised Statutes, the BHA is implementing statewide grievance tracking, transparency, and referral mechanisms.

2. WHAT IS A GRIEVANCE?

​

A grievance is any complaint about Elk’s services, staff, conduct, or policies—excluding decisions comprised under formal appeal or denial processes. For example:

  • Long wait times for services

  • Staff behavior or communication concerns

  • Confidentiality or privacy issues

  • Accessibility barriers or service availability

3. HOW TO FILE A GRIEVANCE

​

You or an authorized representative can submit a grievance in any of the following ways:

  • Phone: Call our Grievance Coordinator at 719. 452. 0393.

  • Email: Send your grievance to connect@enduringconnections.org.

  • In Writing: Send a letter or grievance form to:

All submissions should include your name, contact information, date of concern, description of the issue, and proposed resolution, if applicable.

4. LANGUAGE ACCESS AND SUPPORT

​

If you need help filing a grievance, please let us know. We usually are able to provide interpreter services, assistance for individuals with disabilities, and appropriate referrals if we lack resources of qualified staff.

​

5. WHAT TO EXPECT AFTER FILING

​

  1. Acknowledgment: We'll confirm receipt of your grievance within 10 business days.

  2. Review & Investigation: We'll investigate thoroughly and aim to resolve issues within 30 calendar days.

  3. Updates: We'll keep you informed of progress and provide the outcome in writing.

​

6. ESCALATION & BHA INVOLVEMENT

​

If you're unsatisfied with our response, you may:

  • Request an internal review by Elk’s leadership.

  • Contact the Behavioral Health Ombudsman via BHA:  call 303-866-2789 or email the Ombudsman at ombuds@bhoco.org.

7. CONFIDENTIALITY & NON-RETALIATION

​

Your grievance is treated respectfully and confidentially under HIPAA and applicable Colorado regulations. Filing a grievance will not affect the services you receive. Retaliation is strictly prohibited.

​

8. UPDATING THE POLICY

​

Elk reviews and updates this policy annually, or sooner if required by law or regulation. Any changes will be posted to our website and provided upon request.
